This course describes the guiding principles for managing multiple chronic conditions in home care, identifies the risk factors for coronary heart disease (CHD) and type 2 diabetes mellitus (DM) in older adults, and explores how the pharmacologic management differs in older adults compared to younger adults.

This course discusses interprofessional collaborative practice in the home care setting, the reasons why core competencies across professions is needed and the methods of communication amongst team members.
