HIGN envisions healthcare as a team of professionals, direct caregivers, and patients and their families. Within that paradigm, we see healthcare as a continuum including acute care, primary care, home care, sub-acute care, skilled nursing care, PACE sites, adult day health centers, senior housing and other community-based organizations and the people living in those communities. Our work includes developing resources for all those segments to improve the healthcare of older adults, facilitate transitions of care, palliative care and improve population health by helping people to have the resources to improve health behaviors and mitigate the impact of social determinants of health.
