NextGen Aging is an initiative to improve the care of older adults in the Bronx by expanding the capacity of the healthcare workforce to provide age-friendly and dementia-friendly care through education, training, and mentorship across the care continuum. 

This initiative supports an easy-to-navigate referral process from primary care to community-based services for issues related to social determinants of health, as well as support for people living with dementia. 

NextGen Aging is a partnership between HIGN at NYU Meyers, Montefiore Health System, and JASA, a community-based aging services organization.
