Urinary Incontinence

1. Nursing staff will utilize comprehensive assessments and implement evidence-based management strategies for patients identified with UI.

2. Nursing staff will collaborate with interprofessional team members to identify and document type of UI.

3. Patients with UI will not have UI-associated complications.

To facilitate the implementation of comprehensive and evidence-based transitional care services as patients transfer between different locations or different levels of care within the same location.

Substance Misuse and Alcohol Use Disorders

To implement best nursing practices in older persons with drug, alcohol, tobacco, or other drug use disorders.

Sleep

Older adults will maintain an optimal state of alertness while awake and optimal quality and quantity of sleep during their preferred sleep period.
