Description
This course discusses interprofessional collaborative practice in the home care setting and why interprofessional education (IPE) is important, the reasons why core competencies across professions is needed and the methods of communication amongst team members.
Overview
After completion of the course, practitioners will develop ways to participate in and facilitate interprofessional teams.
This series was developed to assure that interprofessional team members have the knowledge to care for older adults in the home. These modules promote team-based, coordinated, person-centered, evidence-based, quality home care for older adults.
