This series was created to ensure that discharge planners have the knowledge to implement care transition plans for older adults across the spectrum of long-term care. These courses promote team-based, coordinated, person-centered, evidence-based care for older adults in the long-term care setting.

Building a Community Based Chronic Disease Self-Management Program- A Case Study

This module describes why chronic diseases are a community problem. We will describe the importance of self-management in health care, explain how self-management of chronic disease can be improved, and define ways to build healthy communities.
