This series was developed to assure that home care nurses have the knowledge to care for older adults who wish to remain living in their home. These courses promote team-based, coordinated, person-centered, evidence-based, quality home care for older adults.

The courses in this series can be completed individually or altogether. Nursing continuing professional development contact hours will be provided for each of the modules.

Individuals who complete this series will be able to:

1. Differentiate between care coordination and disease management.
2. Provide age-sensitive care for common conditions in community dwelling older adults.
3. Recognize the role of care coordination in establishing working partnerships.
4. Implement plans of care that reflect person and family-centeredness.
