COA-HC-RN: Health Equity and Older Adults in Home Care
This course will help nurses understand key concepts of population health, social determinants of health, and health literacy, and recognize their role in addressing these concepts within the home health setting.
COA-HC-RN: Interprofessional Collaborative Practice
This course discusses interprofessional collaborative practice in the home care setting, the reasons why core competencies across professions is needed and the methods of communication amongst team members.
COA-HC-RN: Managing Chronic Health Conditions in Older Adults
This course describes the guiding principles for managing multiple chronic conditions in home care, identifies the risk factors for coronary heart disease (CHD) and type 2 diabetes mellitus (DM) in older adults, and explores how the pharmacologic management differs in older adults compared to younger adults.
COA-HC-RN: Managing Pain in Older Adults
This course focuses on how to assess and manage pain in older adults as part of an interprofessional team.
