Building the geriatric capacity of the primary care workforce in clinical and community settings to address gaps in health care for older adults and maximize patient and family engagement.

The Collaborative, Continuous Care (3C’s) Model” is a project to enhance inter-professional education, leadership, and team building skills for practitioners and students to help address the complexity of medication management for frail older adults in the community.

This project is managed by the Hartford Institute for Geriatric Nursing (HIGN) at New York University College of Nursing, in coordination with the Consortium of New York Geriatric Education Centers (CNYGEC) at the Mount Sinai School of Medicine (MSSM) and the National Nurses Care Consortium (NNCC).
