This series was developed to increase the age-sensitive healthcare knowledge and skills of primary care nurses. These courses promote team-based, coordinated, patient-centered, and evidence-based care that is responsive to the particular needs of older adults.

The modules in this series can be completed individually or altogether. Nursing continuing professional development contact hours will be provided for each of the modules.

Individuals who complete this series will be able to:

1. Describe the uniqueness of the delivery of primary care for older adults.  
2. Incorporates the principles of population health in delivering primary care to older adults.
3. Describe the varying conditions older adults may present with while receiving primary care.
4. Apply the patient and family centered-care model in creating and implementing care plans.
