Course Description
This course discusses the best practices for medication management in home care and identifies medications that are particularly high risk for older adults.
Overview
The course also describes causes for different types of medication adherence problems, and how different home care professionals can help with medication management.
This series was developed to assure that interprofessional team members have the knowledge to care for older adults in the home. These modules promote team-based, coordinated, person-centered, evidence-based, quality home care for older adults.
This series was created by The Hartford Institute of Geriatric Nursing (HIGN) at New York University Rory Meyers College of Nursing, in partnership with the Visiting Nurse Service of New York.
